Scroll down the form to “Additional Information” and in the “Comments” box type, “Gift To” and follow with the name, address, email address and telephone number of the gift recipient.

After submitting the information, a DD24-7 representative will verify your order with you. After the order has been confirmed, we will send a DD24-7 Membership Card directly to the recipient as well as explain the program and answer any questions that they may have.
